- When filling in the request form, Double check your information to be sure it is correct, as this is highly personalized information based on the Time, Date and Place you were born.

- After purchase, I will send you an email containing directions for filling in and emailing to me the In-Take Form I ask all my clients and students to fill in when receiving services from me.
